By completing and submitting this form you hereby give consent for your minor child to take part in the Bartlesville First United Methodist Church activities.  From this date onward you will not hold First United Methodist Church or its staff or sponsors responsible for an accident or injury to your child.

As our Privacy Policy contends, this information will be kept private and confident.  We will never share, sell, or rent individual personal information with anyone unless ordered by a court of law. Information submitted to us is only available to our Director of Children's Ministries.

If a medical situation occurs that involves your child, we will make every effort to contact you via cell phone or telephone.  If emergency medical attention is warranted, we will contact Jane Phillips Medical Center Emergency Services (911).
